Code of Conduct Agreement

NGTest Academy CODE OF CONDUCT AND GOOD BEHAVIOUR AGREEMENT This agreement covers every player who represents NGTest Academy and the parent, guardian or carer who signs it on the player's behalf. It applies at training, at matches, at tournaments, at club events and anywhere a player or supporter is representing the club. 1. Respect for people Treat every player, coach, volunteer, official, opponent and supporter with respect. Never use abusive, threatening, bullying, discriminatory or harassing language or behaviour on or off the field. 2. Respect for officials Accept the decisions of referees, umpires and match officials. Do not argue with, intimidate or approach an official in a confrontational way during or after a match. 3. Sideline behaviour Parents, guardians and carers agree to support positively from the sideline, to never coach from the boundary, and to never criticise a player, coach or official in front of children. 4. Safety and fair play Play within the laws of the game. Never play in a way that risks injury to another person. Follow all safety directions given by the club, the coach and the venue. 5. Commitment and communication Attend training and matches as often as possible, arrive on time, and let the coach know in advance if the player cannot attend. Wear the correct kit and bring the required equipment. 6. Alcohol, drugs and smoking Players must not consume alcohol, tobacco or any illegal substance while representing the club. This applies at training, matches, tournaments and club events. 7. Social media and photos Think before posting. Never post content that is abusive, negative or disrespectful about a player, coach, official, opponent or the club. Follow the club photo and media policy. 8. Club property and facilities Look after club equipment, change rooms, grounds and facilities. Report any damage to the club as soon as possible. 9. What happens if this agreement is broken If this agreement is broken, NGTest Academy may speak with the player and their family, and may apply any consequence set out in the club's member protection or disciplinary policy, up to and including suspension or removal from the club. 10. What we ask of you as a parent, guardian or carer Support your child, support the coaches and volunteers, and raise any concern directly with the club rather than on the sideline or on social media. Help keep the club a place every child wants to be. This document is a general template provided as a starting point and is not legal advice. Clubs should review it before publishing and adapt it to their own rules and their own member protection policy.

Required Consent

This acceptance is required to complete the form.

Optional Photo and Media Consent

I consent to photographs and video of the Player being captured and used by NGTest Academy for academy communications, website, promotional material and social media.

This consent is optional and may be withdrawn in writing.
