REGISTRATION TERMS AND CONDITIONS
By submitting this registration, the parent or legal guardian named below (the "Guardian") acknowledges and agrees to the following on behalf of themselves and the registered player (the "Player").
1. Participation and Assumption of Risk
The Guardian understands that football, training, tournaments and related sporting activities involve inherent risks, including the risk of injury, illness, accident and property damage.
The Guardian voluntarily permits the Player to participate in NGTest Academy activities and acknowledges the normal risks associated with participation in sport.
NGTest Academy will take reasonable steps to provide a safe environment but cannot guarantee that injuries or accidents will not occur.
2. Medical and Emergency Assistance
The Guardian authorises NGTest Academy, its coaches, officials and volunteers to provide reasonable first aid to the Player when required.
Where reasonably necessary, the Guardian also authorises NGTest Academy to contact emergency services and arrange or seek urgent medical assistance for the Player.
NGTest Academy will make reasonable attempts to contact the Guardian or nominated emergency contact as soon as practicable.
The Guardian understands that medical treatment will be provided by appropriately qualified medical or emergency personnel where required.
Any ambulance, hospital, medical or other treatment costs incurred are the responsibility of the Guardian unless otherwise covered by insurance or applicable law.
3. Medical Information
The Guardian agrees to provide accurate information about any known medical conditions, allergies, injuries, medications or other circumstances that may affect the Player's ability to participate safely.
The Guardian agrees to notify NGTest Academy if any relevant medical information changes.
The Guardian understands that failure to disclose important medical information may affect NGTest Academy's ability to respond appropriately in an emergency.
4. Fitness to Participate
The Guardian confirms that, to the best of their knowledge, the Player is physically capable of participating in the registered activity unless NGTest Academy has been advised otherwise.
If NGTest Academy reasonably believes that continuing to participate may place the Player or another person at risk, the Player may be asked to stop participating until it is considered safe for them to return.
5. Code of Conduct
The Guardian and Player agree to behave respectfully toward players, coaches, officials, volunteers, spectators, facilities and other participants.
The Guardian and Player agree to follow reasonable instructions from NGTest Academy coaches and officials and comply with the rules of the relevant competition, tournament or venue.
NGTest Academy reserves the right to remove or suspend a Player from an activity where serious or repeated inappropriate behaviour creates a safety issue or significantly disrupts the activity.
6. Photos and Video
Photographs and video of the Player may only be used for NGTest Academy promotional, website, social media or academy communication purposes where the Guardian has provided the separate photo and media consent below.
Providing photo and media consent is optional and is not a condition of participation.
The Guardian may withdraw photo and media consent by contacting NGTest Academy in writing. Withdrawal will apply to future use where reasonably practicable and may not apply to material that has already been published or distributed.
7. Privacy
Personal information collected during registration will be used for purposes including registration, communication, administration, player safety, competition management and operation of NGTest Academy activities.
Personal and medical information will be handled in accordance with applicable privacy laws and NGTest Academy's privacy policy.
Information may be provided to coaches, officials, competition organisers, medical personnel or emergency services where reasonably necessary for the Player's participation, safety or administration of the activity.
NGTest Academy will not sell the Player's or Guardian's personal information to third parties.
8. Fees, Cancellations and Refunds
Any applicable registration, tournament or participation fees must be paid in accordance with the payment terms provided at registration.
Where a Player withdraws after registration, any refund will be subject to NGTest Academy's applicable cancellation and refund policy and any costs that have already been committed to competition organisers, venues, equipment, accommodation or other third parties.
If an activity is cancelled or changed due to weather, venue availability, competition decisions or circumstances outside NGTest Academy's reasonable control, NGTest Academy will communicate any available alternative arrangements, credits or refunds where applicable.
9. Personal Property
Players are responsible for their own clothing, equipment, electronic devices and other personal belongings.
NGTest Academy is not responsible for lost, stolen or damaged personal property except where responsibility cannot legally be excluded.
10. Liability and Consumer Rights
Nothing in these Terms and Conditions is intended to exclude, restrict or modify any rights or remedies that cannot legally be excluded under Australian law.
To the extent permitted by law, NGTest Academy is not responsible for loss, damage or injury resulting from the ordinary and inherent risks associated with participation in football and related sporting activities.
11. Guardian Authority
The person completing this registration confirms that they are the Player's parent or legal guardian, or are otherwise legally authorised to register the Player and provide the consents contained in these Terms and Conditions.
The Guardian confirms that the information provided during registration is accurate and complete to the best of their knowledge.
12. Changes to Activities
The Guardian understands that training schedules, tournament times, venues, team allocations, coaches and other activity details may change where reasonably necessary.
NGTest Academy will make reasonable efforts to notify Guardians of significant changes.
13. Governing Law
These Terms and Conditions are governed by the laws of Victoria, Australia.
